Bộ hồ sơ đề nghị cấp chứng chỉ tiêu chuẩn 100 theo OEKO-TEX®

Công ty có nhu cầu làm chứng chỉ OEKO-TEX® cho sản phẩm dệt may, có thể tải bộ hồ sơ xuống để tham khảo và thực hiện các bước sau đây: 

Bộ hồ sơ cấp chứng chỉ OEKO-TEX® bao gồm: 

  • Application (Trang 21-25 application để trống và điền vào file excel thay thế)
  •  Declaration of Conformity
  • Service contract (phần tô vàng)
  •  File Production.infosheet. (Lưu ý: tên thuốc nhuộm, hóa chất sử dụng phải chính xác với chứng từ mua hàng; nếu mua hàng qua công ty thương mại thì phải liệt kê đầy đủ)

Chứng chỉ có giá trị trong vòng 1 năm và sau mỗi năm sẽ được gia hạn tuỳ theo nhu cầu.

Về thời gian, kể từ khi nhận được mẫu và đầy đủ hồ sơ, 4-6 tuần sau đó Công ty sẽ nhận được chứng chỉ.

Quy Trình cấp chứng chỉ:

  1. Quy Trình cấp chứng chỉ:
    1. Khách hàng điền thông tin vào hồ sơ + gủi kèm chứng từ mua hàng của các nhà cung cấp
    2. Văn phòng đại diện kiểm tra hồ sơ và hướng dẫn lấy mẫu cho khách hàng
    3. Khách hàng chuẩn bị mẫu, sau đó gửi hồ sơ có chữ kí đóng dấu scan qua email để lấy mã đơn; bản cứng hồ sơ + mẫu về văn phòng đại diện
    4. Sau 3-4 tuần, Viện kiểm nghiệm duyệt lại hồ sơ và xuất hóa đơn
    5. Khách hàng thanh toán
    6. Quá trình kiểm nghiệm mẫu trong vòng 2 tuần
    – Nếu tất cả các kết quả kiểm đều đạt, xuất chứng chỉ cho khách hàng.
    – Nếu phát hiện chỉ tiêu không đạt, khách hàng gửi mẫu để tái kiểm.

 

Please full-fill all documents then send back to me for checking:
· Application (please leave page 21-25 blank then use excel file instead)
· Declaration of Conformity
· Service contract (yellow highlight part)
· File Production.infosheet. (all dyestuffs, chemicals for Oeko-tex production must be listed; if purchasing through a trader, please detail.)

Total fee = License fee (950 Euro/year) + Audit fee (900 Euro / 3 years) + Testing fee (it will be informed once we receive samples and documents)
If all materials are pre-certified with STD 100 at the equivalent product class and annex + there is no wet/chemical treatment during production, the total fee for the first certification is 2031.50 €.

The validity of the certificate is 1 year. After that, you can renew your certificate.

It normally takes 4-6 weeks for the whole process since we receive sufficient samples and original documents.
Certification process:
1. Applicant full-fills all application + packing list from listed suppliers should be provided accordingly
2. The person in charge checks it then instruct to take the sample
3. All signed & stamped document are sent via email to get order number beforehand; Client dispatches original application with signature and sample to rep office.
4. After 3-4 weeks, the Institute issues API and service contract
5. Client makes payment
6. Testing sample in 2 weeks
– all the tests are passed –> issue draft –> client confirms –> the Institute issues certificate
– Fail result –> sample for retest by client –> retest –> payment for retest (if any) –> issue draft –> client confirms –> the Institute issues certificate
 
NOTE: All materials for Oeko-tex production must be listed and comprised of samples for testing.
For fluorescent colour and special colour (gold, silver…), please list down in application so that we can guide you prepare samples for testing.
